The following informational brochures, documents and forms have been developed to assist litigants, lawyers and judicial officers in applying the rules.
Frequently Asked Questions: A summary of questions & answers for the general public on access to court records.
Litigant Fact Sheet: A fact sheet for civil litigants describing procedures to protect sensitive information that is not protected by state law or rule.
General Sensitive Information Sheet: A form that can be used to file and segregate the sensitive personal information identified in 4.50 (c) in cases that would not be publicly available without cause.
Motions to Seal: A packet of motions and corresponding orders particularly for domestic relation cases to protect health care records, financial records and confidential reports not currently protected in statute. These motions and orders are designed to ensure that the court record (register of action) shows specifically what type of information is included in the sealing request without disclosing the contents of the information.
Confidential Law Enforcement Sheet: This form can be used with petitions for an order of protection to facilitate the service of orders and the entry of records into information systems used by law enforcement agencies, e.g., the FBI’s National Crime Information Center (NCIC).
